• This form is to be used by 1st time EKU new undergraduate students, new transfer students, or new graduate students.
• The student is stating that they have changed their mind, is declining EKU's offer of admission, and will not attend EKU, and therefore requests removal of their registration.
• If the semester in which the student was registered has already begun, or passed, the student is affirming that s/he is not and never has attended EKU classes during the term specified.
• Approval of the form requires confirmation by instructors that the student has never attended class.
